Coordinated Care Network

CCN is a 340B Contracted Pharmacy that provides specialty mail-order services to 340B Covered Entities and their patients. They work with non-profit hospitals, managed care organizations and clinics to offer a true safety-net pharmacy and patient medication management solution. CCN was formed in 1996 by a network of safety-net providers and was originally funded by the U.S. Department of Health and Human Services to build its 340B program and specialty pharmacy. CCN also provides 340B consulting services for Covered Entities, state governments and Medicaid Managed Care Organizations.

The objective of this project was to create a campaign that targets 340B clinics and covered entities.  The intent was to showcase the benefits and ease of using a pharmacy that provides full-service pharmacy management solutions. 

We created three master patient education campaigns and an overall company brochure to educate the clinics on the services and support Coordinated Care Network provides. 

From there, once the clinic was onboarded, we customized the collateral for each clinic with their logo, phone number and language preference, which educated the patients on how to use Coordinated Care Network’s full-service pharmacy management solutions. 

CCN achieved a huge success with this initiative. They have on-boarded more than 200 clinics since its inception, and continue with it today.
